Request for Financial Assistance

The FOTHSCI Board has set aside funds to assist our choir families with choir-related expenses should the need arise.  All forms must be completed by a parent/guardian; requests made by students will not be considered.  Use only one (1) of the following form options to request financial assistance:

Option 1 – Download and complete this form by hand; form must be signed and emailed or mailed –  FOTHSCI Request for Financial Assistance (PDF)  (Note: Unsigned forms will not be processed)

Option 2 – Web version — Complete and submit this form online – FOTHSCI Request for Financial Assistance (Web) (Note: Only completed and electronically-signed requests will be considered)
